Hospital discharge care is short-term support to help someone transition from hospital back to home safely. It can include settling-in support, mobility help, medication prompts, meal prep, personal care, and reassurance during recovery.

Our aim: A safe, supported transition home — with practical help that builds confidence and comfort.

We tailor support to the discharge plan, recovery needs, and family circumstances.

Hospital discharge support may be suitable if someone:

Medication: We can provide medication prompts and routine reminders as agreed in the care plan. If specialist medication administration is required, we’ll confirm what is appropriate during assessment.

Support duration and options

Discharge support can be arranged in a way that fits recovery needs and family circumstances.

  • Immediate discharge support — help on the day of discharge and the first 24–72 hours
  • Short-term recovery support — daily visits for a set period
  • Step-down support — reduce support as independence improves
  • Overnight support — waking or sleeping night care where needed
Flexible: We can adapt support as the person gets stronger, and review the plan as needed.
